Instruction Sheet: Completing the Florida EL1/2S Follow Up Form for ECG Cardiac Clearance

Cardiac clearance is required for all incoming 9th-grade student-athletes (2026-27) and any student in grades 10–12 who has never participated in an FHSAA-sanctioned sport, satisfying the requirement of Florida's “Second Chance Act”. This instruction sheet explains how to complete the EL1/2S Form.

When to Complete the EL1/2S Form

Parents must complete the EL1/2S form if their child's ECG results are interpreted by the cardiologist as abnormal based on the utilizing The International Criteria for Electrocardiogram Interpretation in Athletes. 

Who We Play For (WWPF) ECG Results should be made available within 10 business days of the ECG date.  A Heart Health Advocate (HHA) of WWPF will call the parent/guardian phone number provided at registration within 24 hours to provide comprehensive information associated with the interpretation, as well as regional follow up options. The HHA will leave a voicemail and text message if the parent/guardian does not answer the phone call, requesting a call back.  An SMS text from 1+ (888) 906-9973 will then be sent to the phone number provided during registration with a link to Who We Play For’s secure results portal (results.whoweplayfor.org) and instructions on how to access the portal. 

Who We Play For ECG Results prior to 2026 would have been received from results@whoweplayfor.org with a copy of the ECG, a one-page PDF and a list of regional follow up providers

EL1/2S Form Sections and Meaning

  • Flagged ECG Result (Requires Follow-up)

    • EL1/2S :If the ECG is flagged and requires follow-up, the student will need to take the EL1/2S form (pictured below) to a pediatric cardiologist. 

    • Initial Parent Responsibility (Demographics)
      Parents must complete the “Student Information” section highlighted below.

  • The cardiologist will complete the remaining fields of the form 

    • “Referred for” and below.

Final Submission

Submit the final paperwork to the Athletic Clearance portal or the child's high school administrator.
