1. Introduction and Scope 

Who We Play For, Inc., a Florida nonprofit corporation ("Who We Play For," "WWPF," "we," "us," or "our"), operates electrocardiogram (ECG) heart screening programs and the Who We Play For results portal, including its login pages, result views, downloadable reports and letters, and related content and functionality (together, the "Portal"). This Privacy Policy explains what personal information we collect in connection with screening registration, screening events, the Portal, and related communications, how we use and protect it, and the choices you have. It should be read together with our Terms of Service, available at [Terms of Service URL], which govern use of the Portal. We comply with applicable state and federal laws governing the privacy and security of health information. 

2. Information We Collect 

Information you provide. When you register a participant for a screening or use the Portal, we collect the information you provide so we can provide this service to you, such as: the participant's name and demographic information; the name, relationship, email address, and mobile phone number of the parent or legal guardian completing registration, or of an adult participant registering for themselves; health history information provided during registration or at the screening; and consent, authorization, and signature records. If you contact us, we also collect the contents of your message and the contact information you provide. 

Screening information. A screening creates health information about the participant. This includes the ECG recording and measurements, the screening result, the reviewing physician's interpretation and notes, and the reports and letters generated from the screening, along with the event, location, and date of the screening. 

Information collected automatically. When you use the Portal, we and our service providers automatically collect standard log and device information, such as IP address, browser and device type, pages viewed, and access times. The Portal uses cookies only where needed for it to work, such as session and security cookies. 

Information from screening partners. When a screening is scheduled or performed with a partner organization, such as a laboratory, clinic, school, or community program, we may receive registration and scheduling information from that partner, such as appointment details and confirmation codes used to match the participant to the correct screening record. 

3. How We Use Information 

We use personal information to: operate and administer screening programs, including scheduling screenings and matching each participant to the correct screening record; review and interpret screenings and generate results, reports, and letters; deliver results through the Portal; send notifications that results are available and deliver one-time passcodes used to log in; support follow-up outreach when a result recommends further evaluation; respond to questions and requests; keep the Portal secure, including detecting and preventing unauthorized access; operate, evaluate, and improve our programs; and comply with legal obligations.

4. How We Share Information 

We do not sell personal information, and we do not share personal information with third parties for their own marketing. We share personal information only as described in this section. 

Service providers. We work with trusted vendors that help us run the program, such as companies that host our systems, build and support the Portal, send our emails and text messages, and operate the ECG devices and data tools used in screenings. These vendors may use personal information only to do work for us, and they sign written contracts that require them to protect it. 

Medical professionals. Screening results are reviewed and interpreted by qualified physicians and other clinical reviewers participating in the screening program. 

Screening partners. When a screening is scheduled or performed with a partner organization, we may exchange information with that partner as needed to schedule and conduct the screening and to administer the program. 

Authorized representatives. Where the Terms of Service permit, an authorized representative may access a participant's results based on consent provided by the participant or the participant's parent or legal guardian. 

Legal and safety. We may share information if the law requires it, or if needed to protect the rights or safety of a participant, the public, or Who We Play For. 

Program transfers. If the screening program or the Portal is ever transferred to another organization, for example as part of a reorganization, personal information may be transferred with it and will remain subject to this Privacy Policy. 

At your direction. We share information when you ask us to or when you consent, such as when you direct us to provide a report to a healthcare provider. 

5. Text Messaging Privacy 

Mobile phone numbers and text messaging opt-in information collected for the SMS Program described in Section 7 of the Terms of Service are used to send the message categories described there, such as results notifications, login passcodes, and service messages about a screening. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except for vendors acting on our behalf solely to deliver those messages. You may opt out at any time by replying STOP, and you may reply HELP for help, as described in Section 7 of the Terms of Service. 

6. How We Protect Information 

We use administrative, technical, and physical safeguards designed to protect personal information. These include encryption of information in transit, one-time passcode login, download links that expire after a limited time, and access limits so that people and vendors see only the information they need for their roles. No website or transmission method is completely secure, so we cannot guarantee absolute security. If you believe your access has been used without authorization, notify us promptly at info@whoweplayfor.org

7. Data Retention 

We retain personal information for as long as needed to provide these services and as required by applicable state and federal law.

8. Children's Privacy 

Our screening programs serve participants who are under 18. We collect participant information from a parent or legal guardian as part of registration and screening, with the parent or guardian's consent. The Portal itself is intended for use by adults: parents and legal guardians of screened participants, and screened participants who are at least 18 years old. Participants under 18 are not permitted to access the Portal directly. If you believe a minor has provided information in a manner inconsistent with this section, contact us at info@whoweplayfor.org and we will take appropriate steps to address it. 

9. Your Choices and Rights 

Access and corrections. Results, reports, and letters for your participant are available through the Portal. To request a copy of participant information or a correction to it, contact us at info@whoweplayfor.org

Email. Email is the primary channel for results notifications and login codes. You may update your email address by contacting us. If you withdraw consent to email, we may not be able to deliver results notifications and login codes, so please contact us to arrange another way to receive results, as described in Section 6 of the Terms of Service. 

Text messages. You may opt out of text messages at any time by replying STOP, and you may reply HELP for help, as described in Section 7 of the Terms of Service. Opting out of text messages does not affect delivery by email. 

Additional rights. Depending on applicable law, you may have additional rights with respect to your information. To ask a question or make a privacy request, contact us using the information in Section 12, and we will respond consistent with applicable law. 

10. Third Party Websites 

The Portal and our communications may link to third party websites, such as resources for locating a cardiologist. This Privacy Policy does not apply to third party websites or services, and we are not responsible for their privacy practices. 

11. Changes to This Privacy Policy 

We may update this Privacy Policy from time to time. If we make material changes, we will post the updated policy, update the Effective Date above, and, where appropriate, provide additional notice. Your continued use of the Portal after changes take effect means the updated policy applies.
