Parent Guide: Completing the FHSAA EL01 Form

Overview

To participate in sports for the 2026-2027 school year, Florida's Second Chance Act requires student-athletes to complete an electrocardiogram (ECG) screening. The EL01 – Electrocardiogram (ECG) Screening Form is the official document used to record these results and must be submitted to your child's school for athletic clearance.

Step 1: Student Information

At the top of the EL01 form, please complete the Student Information section in its entirety. This ensures the screening results are correctly associated with your child's school records.

Step 2: Interpreting & Completing Results

The completion of the EL01 form depends on the specific interpretation of your child’s ECG results:

●  Section A (Normal/Low Risk): If the ECG results are interpreted as normal or low risk, the parent or guardian must complete Section A of the EL01 form to certify the results.

● Part B (Medical Eligibility/Follow-up): If the ECG result is flagged as abnormal or requires additional evaluation, your child cannot participate in sports until they receive written medical clearance. Part B is the specific section that must be completed and signed by a licensed medical practitioner (such as a pediatric cardiologist) following a further clinical evaluation.

Results from Private Medical Offices

If your child’s ECG was performed at a private medical office rather than through a Who We Play For community event, the same state requirements apply. The results must still be recorded on the official EL01 form and must be signed by a licensed medical practitioner trained in the management of ECG results to satisfy athletic clearance requirements.

Important Clarifications

Results window: If you attended a Who We Play For screening event, results are typically delivered via email within 10 business days. If you do not receive them within this window, please check your spam folder or request results at whoweplayfor.org/results.

Other Requirements

In addition to the EL01 form, all student-athletes must also submit the EL02 – Pre-Participation Physical Evaluation form. Both documents are necessary to satisfy the requirements of the Second Chance Act.

Final Submission

Once all sections are finished and any necessary medical signatures are obtained, upload the completed paperwork to your school's Athletic Clearance portal or submit it directly to your high school administrator to ensure your student is cleared for participation.
